Oh, fine, I'll bite:

After tuning guitar, forgot to disengage tuner, which muted guitar for the whole first half of song until the other guitarist noticed it was still on.

Selected wrong patch for a solo, resulting not in a smooth, lead tone, but a clean, phaser tone.

After switching guitar, did not realize that strap had caught on tremolo bar, raising pitch for first 10 seconds of song. Other guitarist caught this one, too - think I'll keep him!

Forgot lyrics to many well-known covers many times. People just think I'm making new ones and whatnot!

I'm sure there's more, but that's all my brain will let me remember for now!

The bassist in my band blew the fuse in his amp after the first song.
he had to go direct into the board for the rest of the show.

not really an epic fail...but a fail nonetheless

That reminds me... I really need to get some extra fuses... and tubes for that matter.

During my fist bands first show, I started playing a song in the wrong key, and was so sure I was right and got the bassist and player to start playing in the key, half way thru when the chorus came up I realized I was wrong (and embarrissed) and we changed to the real key, which sounded awfull, then we ended the song half way thru. Not pretty, luckily is was a very small crowd.
(to my defense I was playing the song in the correct key, just forgot that we had changed the key of the song so the vocals weren't as high in practice).

Epic failure on my part.
